Transcript Selection

Enter the specific tax form number on Line 6. Common examples include Form 1040 for individual returns, Form 1065 for partnerships, Form 1120 for corporations, and Form 1120-S for S corporations. Enter only one tax form number per request. You may request multiple transcript types on a single form by checking multiple boxes, as long as all requests relate to the same tax form number.

Check Box 6a for a return transcript showing your original return information. Available for the current year and the three preceding years.

Check Box 6b for an account transcript showing payments, penalties, and adjustments. Available for most return types.

Check Box 6c for a record of account combining both transcripts. Available for the current year and the three prior tax years. Select this option if you are unsure which type you need.

Tax Year or Period

Enter the ending date of the tax year or period on Line 9 using the month/day/year format. For calendar year 2023, enter December 31, 2023. For quarterly returns, enter each quarter separately.

Signature Requirements

Select the attestation box in the signature area to confirm your authority to request information. Forms without this box checked will not be processed. Sign and date the form exactly as your name appeared on the original return. If you changed your name, sign using both names. Include your phone number.

For joint returns, only one spouse needs to sign unless both are listed on Lines 1a and 2a.

Corporate officers with legal authority, designated individuals from the board of directors, or authorized employees can sign on behalf of businesses. Include your title when signing on behalf of an entity. Partnership members during the tax period may sign on behalf of the partnership.

Representatives must attach Form 2848, showing a specific delegation of authority on Line 5, to sign Form 4506-T. The IRS must receive the form within 120 days of the signature date, or it will be rejected.

To submit by mail or fax, send Form 4506-T to the appropriate IRS address based on your state of residence or business location. Separate address charts are available for individual transcripts and all other types of transcripts.

If you are requesting multiple transcript types with different addresses shown in the charts, send your request to the address corresponding to your most recent return location. Most mailed or faxed requests are processed within ten business days. The IRS will mail transcripts to your record address shown on Line 3.

Since July 2019, the IRS has only mailed transcripts to the taxpayer’s address of record. Third-party mailings are no longer available. If a third party needs direct access, they must work through the Income Verification Express Service program.

Common Errors to Avoid

Ensure your name exactly matches the one on your tax return. Minor variations can cause processing delays or rejections. Verify that addresses are complete and accurate. Missing apartment numbers or incorrect ZIP codes frequently cause issues. Complete all applicable lines before signing. Missing information in Lines 1 through 4 and Line 6 results in rejection.

Mark the attestation checkbox. Forms without this confirmation will not be processed. Enter only one tax form number on Line 6.
