Form 1096, 2021 Tax Year Filing Checklist
Purpose
Form 1096 transmits grouped paper information returns (1097, 1098, 1099, 3921, 3922, 5498, W-2G) to the IRS. For 2021, filers of 250 or more returns of any single type are required to file electronically, unless an approved waiver applies. The Taxpayer First Act of 2019 authorized the Treasury to reduce this threshold, but no regulatory change was finalized for 2021 returns filed in 2022. Forms 1099-QA and 5498-QA must be filed on paper only, regardless of quantity.
Filing Steps for 2021
Step 1: Confirm Paper Filing Eligibility
Verify that you are not required to e-file the $250 return rule. If filing 250 or more returns of any single type, you must file electronically unless you have an approved waiver. The 250-return threshold remains in effect for 2021. Form 1096 is not required for electronic submissions.
Step 2: Group Returns by Form Type
Prepare a separate Form 1096 for each form number. Do not combine different form types (e.g., Form 1098 and Form 1099-A) on a single transmittal. Each group of forms requires its own Form 1096.
Step 3: Select the Correct Form Type Box
Enter an X in only one box in line 6, which corresponds to the form type you are transmitting. Examples: 1099-NEC in box 71, 1099-MISC is in box 95, 1099-K is in box 10, and W-2G is in box 32. Verify the box code matches the forms being sent.
Step 4: Enter Your Taxpayer Identification Number
Enter your EIN in box 1 or SSN in box 2—not both. Sole proprietors without an EIN must use their SSN in box 2. All others use their EIN in box 1. The name, address, and TIN on Form 1096 must exactly match those on each accompanying information return.
Step 5: Enter Total Number of Forms
In box 3, enter the count of completed forms only. Do not include blank, voided, or summary forms. Count individual forms, not pages. Example: if one three-to-a-page sheet contains two completed 1098-E forms, enter 2.
Step 6: Enter Federal Income Tax Withheld
In box 4, enter the total federal income tax withheld from all forms being transmitted. This box applies only to forms reporting withholding (e.g., W-2G, 1099-R). Leave blank if transmitting forms without withholding boxes.
Step 7: Calculate Box 5 Total Amounts
Enter the total of specific boxes for each form type. No entry is required for Forms 1098-T, 1099-A, or 1099-G. For Form 1099-NEC, total box 1 only. For Form 5498, sum boxes 1, 2, 3, 4, 5, 8, 9, 10, 12b, 13a, and 14a. For Form 1099-MISC, sum boxes 1, 2, 3, 5, 6, 8, 9, 10, 11, and 13. Refer to Form 1096 instructions for the complete list of boxes to total for each form type.
Step 8: Sign and Date Under Penalty of Perjury
Sign Form 1096 declaring you have examined the return and accompanying documents. Enter your title in the designated field. The IRS will not accept unsigned forms.
Step 9: Determine the Correct Mailing Address
Mail to the appropriate IRS Submission Processing Center, based on your principal business location or legal residence. Filers located outside the United States should mail to Austin, TX.
Step 10: File by the Correct Deadline
File Form 1096 with Forms 1098, 1099 (except 1099-NEC), 3921, 3922, or W-2G by February 28, 2022. File Form 1096 with Forms 1099-NEC by January 31, 2022. File Form 1096 with Forms 5498 by May 31, 2022.
Step 11: Package Forms Properly
Send forms in a flat mailer without folding. Do not staple forms together. Use only official, IRS-printed, scannable forms for Copy A. Downloaded or photocopied versions of Form 1096 cannot be scanned and may result in penalties.
Step 12: Submit Summary Data Only
Do not include summary or subtotal forms with Form 1096. Only summary data (total forms count, total withholding, total amounts) should appear in boxes 3, 4, and 5. Original and corrected returns of the same type may be submitted together under one Form 1096.
2021 Year-Specific Filing Details
Electronic Filing Threshold
The Taxpayer First Act of 2019 authorized the Treasury to reduce the 250-return electronic filing threshold for 2021 tax returns. The IRS instructed filers to monitor www.irs.gov/Form1099 for regulatory changes that take effect in 2022. No reduction was finalized before the 2021 tax year, so 250 remains the operative threshold for 2021 paper filers.
ABLE Account Forms
Forms 1099-QA and 5498-QA are paper-only returns regardless of the quantity filed. The 2021 instructions state these forms can only be filed on paper due to the low volume of ABLE account returns processed annually.
Combined Original and Corrected Returns
The 2021 General Instructions for Certain Information Returns confirm that originals and corrections of the same form type may be grouped under one Form 1096 transmittal, thereby simplifying the filing process for filers who make corrections.
Box 5 Reporting for Form 5498
Form 5498 filers must total boxes 1, 2, 3, 4, 5, 8, 9, 10, 12b, 13a, and 14a when completing box 5 of Form 1096. These boxes capture IRA contributions, fair market value, and rollover or conversion activity for comprehensive reporting.
Key Reminders
Form 1096 is required only for paper submissions. Electronic filers through the FIRE System do not submit Form 1096. Ensure the filer’s name and TIN match those used on Form 945 to avoid information return penalties.
All information returns filed on paper must be sent to one of three IRS Submission Processing Centers based on specific state assignments, not regional descriptions.
