Employer Identification Requirements

Verify EIN and Complete Identification Fields

  1. Verify your employer identification number (EIN) matches on both Form W-3 and all accompanying Forms W-2 for tax year 2024.
  2. Complete all required employer identification fields: name, address, ZIP code, EIN, and control number in box a.
  3. Enter the total number of Forms W-2 being transmitted in box c; this count must be accurate and match the actual Copy A forms enclosed.

Indicate the kind of employer that best describes your organization: none apply, 501(c) non-government, state/local non-501(c), state/local 501(c), federal government, or third-party sick pay.

The employer type designation helps SSA categorize your filing for proper processing. Select the single category that accurately reflects your organization's legal and tax status for 2024.

Signature and Submission Procedures

Execute Signature Block

An authorized officer must sign the declaration under penalties of perjury on Form W-3. Include the signer's title in the designated field to identify their authority.

Date the form with the actual date of signature for 2024 or early 2025 before the filing deadline.

Prepare Mailing Package

Do not staple Form W-3 to Forms W-2 or any other documents, as staples damage machine-readable forms.
